1.1 AR GL Detail Report
This report enables you to examine receivables
posted to General Ledger in the AR Module. If you did not purchase the
AP and GL modules, you should not run this report.
You can run this report for either one patient
or all patients for a specific month and year.
All transactions posted to the default receivable
accounts, which are set in facility information screen, at the specified
month and year, starting with Balance Forward from previous month and totals
at the end of month will be printed.
The report is sorted by patient name and then
transaction date and displays the following:
Patient name and social security number
Transaction date
Transaction description
Transaction amount
1.2 Cash Receipt Journal
This report enables you to examine payments posted
in the AR module for a specified date range. The report is sorted by patient
name and then date of payment grouped by transaction number and displays
the following:
Transaction number
Patient name
Date of payment
Remittance advice date
Description (comment field when posting
payments)
Amount of payment
Debit and Credit GL account number
1.3 Payments By Type
This report enables you to examine payments posted
in the AR module for a specified date range and type of payment. The report
is sorted by patient name and then date of payment and displays the following:

1.4 Aged outstanding Report
This report enables you to examine your aged
receivable for a specified receivable type and month & year. You can
also run this report for all receivable types(Aging Analysis.) The report
is sorted by patient name and displays the following:
Patient name
Total receivable balance for the patient
Receivable amount aged by 0-30, 31-60,
61-90, 91-120, and over 121
Total receivable amount for the specified
month and year
1.5 Aging Detail Report
This report enables you to examine your aged
receivable for a specified month and year. It will show all receivable
types on same report. You can run this report either for one patient or
all patients. The report is sorted by patient name and displays the following:
Patient name and social security number
Receivable amount aged by current, 30 days, 60
days, 90 days, and 120 days.
Total receivables for patient
Total receivables for facility
1.6 Adjustment Report
This report enables you to examine all adjustments
made in the AR module for specified date range. Adjustments are defined
as all payments posted with the “Adjustment” check box checked at the time
of posting payments. Also any claim which was modified after posting the
claim, through “Adjustment menu” in Claim information screen. The report
is sorted by patient name and then transaction number and displays the
following:
Patient name and social security number
Adjustment date and description
GL account number the adjustment was applied
to
Debit and Credit amount
1.7 Balanced Due For Discharged Patients
This report enables you to examine a list of
patients who are discharged from census, but all of their private receivable
is not collected. The report is sorted by patient name and displays the
following:
Patient name and social security number
Balance due
Comments (if there are any charges left
in charge master for the patient)
Note that the patient should be admitted and
discharged from census in order to show on this report. Part B Medicare
patients who are not admitted in census will not show here if there is
any balance due.
2.1 Patient Income Report
This report enables you to examine a list of
patient income amounts for those who are admitted through census (for Open
admission records). The report is sorted by patient name and displays the
following:
Patient name and social security number
Medicaid number
HIC number (Medicare number)
Income effective date
Income amount
2.2 DMA 6 Ending Date For All Patients
This report enables you to examine a list of
patients whose DMA certification ending date has expired or is within 45
days of expiration from the time you run the report. The report is sorted
by patient name and displays the following:
Patient name and social security number
Medicaid number
DMA certification ending date
2.3 DMA 6 Ending Date For Open Admissions
This report enables you to examine a list of
patients whose DMA certification ending date has expired or is within 45
days of expiration from the time you run the report. This report only displays
patients who are admitted through census and still admitted (Open Admission
records). The report is sorted by patient name and displays the following:
Patient name and social security number
Medicaid number
DMA certification ending date
Admission date
2.4 DMA 59
This report enables you to fill out DMA 59 form
for specific patient. Once you complete the form, you can print it by clicking
on “Print” button. The form will not be saved and it should be printed
once it is complete.
3.1 Admission Report (Face Sheet)
This report will show specific patient admission
records on screen. If patient has more than one admission record (re-admitted),
you are able to view information from previous admissions. To print face
sheet, Click on print button on first page of this report.
3.2 Admission Summary Report
This report enables you to examine a list of
patients admitted during specified date range. The report is sorted by
admission date and then patient name and displays the following:
Admission date
Patient name and social security number
3.3 Patient Admission History Report
This report enables you to examine a list of
all patients on file along with their admission history. The report is
sorted by patient name and displays the following:
Patient name and social security number
Admission date
Payer source
Status
Transaction dates
3.4 Daily Census Report
This report enables you to examine a list of
patients and their payer source for specified month and year. You can run
the report for one payer at the time or for all payers. The report is sorted
by patient name and displays the following:
Patient name and social security number
Payer source for every day of the specified
month and year
3.5 Patient Days Summary
This report will print a summary of patient days
of all payers for specified date range. The report displays the following:
Payer type
Number of patient days for each payer type
Number of hospital days for each payer
type
Total number of days for each payer type
Total number of days for facility
3.6 Print Charge Master
This report enables you to examine a list of
charges in charge master for either specified date range or all charges
in charge master. You could also choose specific revenue code or all revenues.
The report is sorted by patient name and then revenue code and displays
the following:
Patient name and social security number
Revenue code
Revenue description
Service date
Reimb. (Refer to entering charges to charge
master for definition)
Rate
Units
Total of charge amount
3.7 Discharge Summary Report
This report enables you to examine a list of
patients discharged during specified date range. The report is sorted by
discharge date and then patient name and displays the following:
Discharge date
Patient name and social security number
4.1 Room/Bed Database Report
This report enables you to examine a list of
rooms & beds and the patients assigned to them The report is sorted
by room & bed and displays the following:
Room
Bed
Certified
Occupied
Rate
Patient name
4.2 Room/Bed Assignment Report
This report enables you to examine a list of
rooms & beds and the patients assigned to them. This report only displays
the occupied beds. The report is sorted by room & bed and displays
the following:
Room
Bed
Certified
Patient name
4.3 Room/Bed History Report
This report enables you to examine a list of
patients and all of the room & bed assignments for each patient since
admission date (Only patients with open admission record). The report
is sorted by patient and effective date and displays the following:
Patient name and social security number
Admission Date
Effective Date
Room
Bed
Bed Rate
4.4 Certified Bed Occupancy Report
This report enables you to examine the number
of days that certified beds were occupied by non-medicare patients for
a specified date range. The report is grouped by payer source and
displays the following:
Patient name and social security number
Admission Date
First Day of Bed Occupancy
Last Day of Bed Occupancy
Room
Bed
Certified
Payer Source Begin Date
Payer Source End Date

4.5 Physician Patient Report
This report enables you to examine a list of
physicians and the patients in their care. The report is grouped
by physician UPIN number then patient name and displays the following:
Physician UPIN number
Physician Name
Patient Name
Room
Bed
Admission date
Patient social security number
Patient HIC Number (Medicare number)
Patient Medicaid Number
Patient date of birth
5.1 Part A RUG Category Report
This report enables you to examine a list of
patient’s RUG scores for a specified date range. The report is sorted
by Patient Name and displays the following:
Patient Name
Claim Service Dates
Assessment Date
RUG category
Modifier
Units
Rate
Total price
5.2 Part A RUG Summary Report
This report enables you to examine the number
of units per RUG Score for 12 months along with a total for the RUG Score
and a grand total for each month beginning with a user specified month.
The report is sorted by RUG category and displays the following:

5.3 Bad Debt Report
For the state of Oklahoma:
This report enables you to examine the amount
of Part A & Part B Medicare bad debt for a specified date range where
Claims co-insurance bill dates are 121 days before the specified date range.
For example if you run this report for 7/1999 until 6/2000, then all outstanding
co-insurance amounts for claims which the co-insurance bill dates are between
3/2/1999 (121 days before 7/1/1999) and 3/1/2000 (121 days before 6/30/2000)
will show on report. The report is sorted by patient name and displays
the following:
Patient name
HIC number (Medicare number)
Claim service dates
Co-insurance amount
Type (M-Medicaid , P-Private)
Medicaid number, if type is M
Co-insurance bill date
Date of write off
Amount of write off
For all other states:
This report enables you to examine the amount
of Part A Medicare bad debt for a specified date range where Claim service
dates are within the specified date range. The report is grouped
by year, month and sorted by patient name and displays the following:
Patient Name
Medicare Number
Medicaid Number
Claim Service Dates
Coinsurance Days
Medicare Bill date
Medicare Date of Payment
Coinsurance Amount
Patient Liability
Medicaid Payment Amount
Amount of Write-off
Write-off Date
5.4 Part B Therapy Bill to Date Report
This report enables you to examine the Medicare
Part B Therapy charges for a specified year. The report is sorted by patient
name and displays the following:
Patient name and social security number
PT/ST charges
OT charges
5.5 Part B HCPC Billing Summary
This report enables you to examine the Medicare
Part B HCPC records for a specified date range. The report is grouped and
sorted by patient name and then claim and displays the following:
Patient name and social security number
Claim service dates
Revenue code
HCPC code
Modifier
Units
HCPC code reimbursement rate
HCPC code charge rate
Total reimbursements for HCPC code
Total charges for HCPC code
The report also provides subtotals for claim,
patient, and grand total for facility.
5.6 Blue Cross MDD Report
For facilities that transmit their Medicare claims
to Blue Cross of Georgia, this report enables them to download their Medicare
MDD report. This option will run Procomm software. A script to download
the MDD report is provided by MedEasy.
5.7 View/Print Blue Cross of Georgia MDD Reports

When the screen opens a diaglog box is displayed
so that you can choose the directory where you downloaded your MDD reports
if you did not setup a default download directory in the Facility screen's
'Links' tab. If you have a default directory setup then the list
of reports will be displayed in the table.

If you want to change the directory click on the
tool pushbutton, locate the directory and click OK.

Select the report that you want to see from the table.
Click the 'View report' pushbutton and the report will be displayed on
screen. To print just click on the pushbutton with the image of a
printer.

To delete a report from your hard disk permanently.
Select the report in the table then click the 'Delete Report' pushbutton
and confirm that you want to delete the report.

Click 'Close' to close the screen.
